Need for customized medication
Last Year 32,000 Ontarians Needed a Lower than Available Dose
The Globe & Mail article reports that last year in Ontario 27% of seniors taking thiazide diuretics
and beta blockers required a lower than commercially available dose.
The need for lower doses of commercially available medications appeared in the academic press as
early as 1991. Writing in the Lancet, Herxheimer noted that some commercially available drugs
may contain as much as 70% more medications than required by the patient.
On average drugs probably contain 25% in excess of that required by the patient.

because this helps market penetration"
The problem becomes critical in the elderly because aging is accompanied by a reduction in cardiac
and renal reserves. In the elderly, physiological adjustments available to the young no longer
function. Thus there is a need for dose titration in order to supply just the correct amount of
medication for a given patient.
Some physicians and patients attempt to solve the problem by cutting tablets in half.
Going from a whole tablet to half a tablet represents a 50% reduction in dosing.
Alternatively going from half a tablet to a whole tablet represents a 100% increase in dosing
